Insulin Resistance Syndrome (IRS)
The hormone insulin regulates sugars in the blood.  When the body does not respond properly to the hormone the blood levels of insulin are elevated and this is called Insulin resistance.  The syndrome is made up of a group of medical conditions that may include obesity, hypertension, high cholesterol, high triglycerides, type 2 Diabetes, and high insulin blood levels.  Although the disease process can remain effectively hidden for years, some clues of this syndrome may be seen as abdominal obesity, fatigue, and intense craving for sweets. 
Pregnancies
With pregnancy and childbearing, the abdominal muscles and the skin are stretched apart and loosened causing the abdomen to protrude and unsightly stretch marks on the skin.  At times hernias can develop with the weakened abdominal muscles.
Dramatic Weight loss
Patients with weight problems suffer from similar conditions as women whom have undergone pregnancies.  In patients who lose this weight, either through dietary modification and exercise or from surgical procedures such as stomach banding, the loss of fat results in a greater amount of loose skin than seen with pregnancy alone.

When the abdominal muscles are bulging and when there is much loose skin then an abdominoplasty is done to remove the excess skin and to tighten the abdominal muscles.  The upper part of the abdomen and the sides should be liposuctioned before or during the abdominoplasty to remove the excess fat from these areas and contour the body.  Stretch marks that are on the lower part of the abdomen can be removed with this procedure.
Mini-Abdominolplasty
When loose skin is present mostly on the lower part of the abdomen, then a shorter incision is made on the lower abdomen and the excess skin is removed.  The scar is more cosmetically acceptable and the recovery time is shorter.
Periumbilical Tuck
In some cases the loosened skin can be removed by cutting around the umbilicus only.  This procedure does not require cutting on the lower abdomen and the scar around the umbilicus tends to heal well and becomes hidden in the umbilicus.
