Surgical and non-surgical treatments for a youthful smooth neck

As we age, we all start to see the “turkey neck” starting to show itself when we look at ourselves in pictures or in the mirror.  Over time, the skin and underlying tissues of the neck sag and become unsightly.  The skin’s appearance tends to look wrinkled, leathery or “chicken-like” with uneven red, brown and dark toned blemishes over time.  The environmental effects such as years of sun exposure and smoking also take their toll on the skin.  Today, we have a much better understanding of the causes of loose and damaged skin of the neck and have technologically advanced approaches to fulfill the desires of most individuals.

Surgical options:

Traditional nip-tuck neck and lower face lift

The traditional neck lift involves cutting the excess skin out and lifting the underlying tissues to tighten the loosened skin on the neck.  This however does not improve the skin texture or discoloration.  The recovery time for this procedure is from 5 to 10 days and has a higher rate of risks and complications associated with it than the rest of the procedures discussed below

Short incision Neck lift

The short incision neck lift is a smaller version of the traditional neck lift surgery reserved for people with less skin laxity involving only the neck.  The recovery time to doing normal daily activities is between 3 to 8 days.

The most effective way to regain a defined neck and jaw line the fullness on the neck and jowls is to remove the fat that causes this problem by liposuction.  Most cases can be done under local anesthesia or under mild sedation.  The incisions are very small and almost undetectable once healed.  The recovery time is from 1 to 3 days and an elastic band is recommended for 1 to 3 weeks at nights to help accelerate recovery and to optimize the results of the procedure.  In most cases, the skin tightens and no other procedure is needed.  In some individuals, the array of options listed in this article can be combined with liposuction to achieve the desired look.

Short Procedures

Anchor Lift

This is the latest of the thread techniques to help lift the neck without the risks and recovery time associated with a surgical neck lift.  Unlike the more friable Fear Lift threads, the Anchor Lift threads are more durable and capable of achieving a neck lift with good results in the appropriate individuals.  The threads are placed and the neck is lifted without visible scars.  The down time is only 1 to 2 days.

Thermage is a radio frequency device designed to tighten the loose skin.  The treatments have shown some results, but inconsistently.  Because the treatments are painful, usually patients need to be given anesthesia and/or pain medication.  The technology is currently being improved by the company to see if results with this treatment can become more predictable.

Lev-Lite vs. Ablative Lasers

Most individuals with wrinkled, discolored, blotchy loose skin may opt for laser treatment to tighten and rejuvenate the skin on the neck and ‘V of the chest’.  Lev-Lite is the most advanced, safest and effective method to improve the skin texture, reduce wrinkles and rejuvenate the skin.  Lev-Lite combines a photosensitizing compound with a non-ablative laser (a laser that does not damage or remove the surface of the skin) to achieve a fast and noticeable improvement in all skin types with minimal recovery time.  Lev-Lite treatments can achieve similar results as the older ablative laser methods without the risks and the long recovery time associated with them.  Ablative lasers that remove the top layers of the skin and leave the skin raw are the CO2 and the Erbium lasers.  These lasers have a high risk of scarring, keloiding and causing skin discolorations on the neck and have long recovery periods of 2 weeks to 4 months.  Lev-Lite is the treatment of choice for rejuvenating the skin on the neck.

Chemical peels

Chemical peels are an effective way to rejuvenate the skin.  The chemical peels vary in their strengths and their side-effect profiles.  For the neck, chemicals peels of moderate strength or below should only be used.  Strong chemical peels are associated with higher side effects.  Depending on the skin type and amount of skin rejuvenation needed, an appropriate chemical peel can be selected to optimize the benefits and minimize the risks.  I have formulated a chemical peel which can be used on all skin colors and on all body parts to rejuvenate the skin.  With the Newman Peel there is no recovery period but you may see some dandruff like flaking of the skin for 2 to 4 days.  A series of 3 to 6 mild to moderate chemical peels 2 to 4 weeks apart can achieve much improvement on the skin texture, color and wrinkles

Mesotherapy treatment entails a series of injections of a combination of compounds to melt the excess fat under the skin and to help the skin to become more firm and rejuvenated.  Some people experience bruising, redness and itching after the treatment.  This treatment is a good option for individuals who need a mild to moderate improvement of their neck.

Botox can treat the neck bands and give a smoother look to the neck.  Those who do not wish to undergo surgery to correct the cords and bands on their neck can benefit from Botox injections.  The results will last for an average of 3 to 4 months.

Non-Surgical Neck Lifts

Titan is the most advanced method of non-surgical skin tightening.  The loose skin of the neck can be noticeably tightened without scars, pain or down time.

Non ablative lasers

Lasers can be used to help reduce the redness, discoloration and to improve the texture of the skin.  These lasers do not remove the top layer of the skin and have little to no recovery time associated with them.

Combined Therapies

Most patients will need a combination of treatments to achieve their desired youthful neck.
